Haven't seen the next ones yet but just wanted to say that there are several investigations I can think of where humans weren't blamed (even if they made a mistake) and the outcome is still policies and procedures. A big one is level crossings - these would be safe if people actually abided by their warnings (and they work properly) but people still make their way on to the tracks and get hit by trains, then it goes around in the media that the crossing is unsafe and should be removed. In situations where the person actually didn't think what they were doing is bad, I can understand the frustration and the crossing should be improved, but there are many cases of people knowingly doing dumb things and then they get more policies and procedures put in place to annoy the rest of us. I feel there has to be an expectation on people to actually want to be safe and as much as that is normally the case there are people who will almost purposely and knowingly do things wrong and there is really no possible way to stop it without stupid policies and procedures.Also there is an element of common sense that has to be expected as well. Just say someone is actually stupid enough to end up in an obviously dangerous situation without realising. What are you meant to do about that. We can't go around building fences on every mountain, sometimes people may just have to learn the hard way.
An interesting topical review of human error. Look at the BA investigation into its recent problems where one person crashed the entire system. Old view or new view?
